Apply for Admission
Our application process begins Nov. 1 for the following school year and involves a few steps:
- Create an account in FACTS, our school information system.
- Verify your email address.
- Log in to FACTS, and begin a New Student Application.
- If relevant, complete an Application for Financial Assistance.
- Submit your application.
- Track your application's status through FACTS anytime.
Admission decisions* typically occur by email on or before Jan. 15. Decisions for applications submitted after Jan. 15 may be rendered within 72 hours.
Once a decision is released, any required next steps will automatically populate within FACTS.
*If your child is placed on a waitlist, we will contact you via email as soon as a spot becomes available.
Additionally, please note:
Only one application is required per family unit per school year. List additional incoming students on the application under "Siblings."
There is a one-time $125 application fee per family.
An application remains valid for one academic year. You may retrieve or print a copy of your completed application at any time by logging into FACTS.

Confirm your acceptance by submitting your $350 tuition deposit.
Deposits are non-refundable and credited towards tuition at completion of the Enrollment process.

Supplemental documents required for transfer students
Kindergarten through 8th Grade Transfer Students should submit a current report card, IEP (if relevant), and a letter of Good Standing from their current school's principal.

Which health and dental forms are required?
- All New Students: Child Health Examination Form, Dental Form
- Kindergarten: Child Health Examination Form, Dental Form, Kindergarten Eye Exam
- Grade 2: Dental Form
- Grades 5, 7, 8: Sports Physical Form
- Grade 6: Child Health Examination, Dental Form

Admission Policy
Notre Dame School does not discriminate on the basis of sex, race, national or ethnic origin in admission of educational policies, athletics, or other school-administered programs.
Our Admission Policy grants application priories:
- Children of Parishioners, defined as students from:
- a. school families who are active members* and are currently enrolled at Notre Dame School.
- b. school families who are active members* and have siblings currently enrolled at Notre Dame School.
- c. parish families who are active members* of Notre Dame parish.
- Transfer students from other Catholic Schools.
- a. From schools that are merging or consolidating.
- b. From schools not offering full Gr. K-8 programs.
- Children of non-parishioners.
*Active members are registered parishioners who demonstrate their commitment to their Catholic faith by regularly attending weekly liturgy, volunteer time or service in a parish or school ministry, and provide regular financial support to the parish.
